I consent to the processing of my personal data by Baltic Neopolis Orchestra Association for purposes related to the organization of Szczecin Classic Academy, in accordance with the Personal Data Protection Act (Journal of Laws of 1997, number 133, item 833 as amended)

If I qualify for participation, I consent to transfer, by contract to the Organizers, the rights to perform artistic works during Szczecin Classic Academy in the following fields of use: 3.1 .: the right to publicly play, perform and broadcast; 3.2 .: the right to record works by any means, including digital, optical, magnetic, print and photographic film; 3.3 .: the right to enter songs into computer memory and into own databases; 3.4 .: the right to adapt the work to the requirements of the Organizers; 3.5 .: the right to use the work for the promotion and advertising of Szczecin Classic Academy; the transfer of the rights referred to in Act 3 is not limited in time or territory, and takes place without remuneration, and will occur as soon as I perform my works during the Szczecin Classic Academy; I renounce the exercise of rights arising from the protection of my personal rights regarding artistic performance
