I voluntarily consent to the processing of my personal data by the Baltic Neopolis Orchestra Association for the purposes of organising the Szczecin Classic Academy, in accordance with the Personal Data Protection Act of 10 May 2018. (as amended), and pursuant to Article 6(1)(a) of the Regulation of the European Parliament of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (RODO).

In the event that I am qualified to participate, I undertake to transfer by contract to the Organisers the rights to the artistic performance of the works created during the Szczecin Classic Academy in the following fields of exploitation: 3.1. the right of public reproduction, performance and broadcasting; 3.2. the right to record the work by any technique, including digital, optical, magnetic print and on photographic film; 3.3. the right to enter the work into computer memory and into own databases; 3.4. the right to adapt the work to the requirements of the Organisers; 3. 5. the right to use the work for the purpose of promotion and advertising of Szczecin Classic Academy; The transfer of rights referred to in item 3 shall not be limited in time or territory and shall take place without compensation and shall occur at the moment of performing the work by me during Szczecin Classic Academy. In order to implement the provisions of para. 3.4. I waive the exercise of my rights resulting from the protection of my personal rights concerning the artistic performance.

I agree to receive electronically from the Data Administrator - Baltic Neopolis Orchestra Association, ul. Wyszyńskiego 27/9, 70-202 Szczecin, information related to my application for participation in the Szczecin Classic Academy, organised by the Administrator.

INFORMATION CLAUSE for participants in the Szczecin Classic Academy:

We inform you that:  1) the controller of the personal data is the Association of Baltic Neopolis Orchestra, Wyszyńskiego Street 27/9, 70-202 Szczecin - Personal Data Administrator 2) the contact for data protection e-mail: biuro@balticneopolis.pl 3) the personal data will be processed in order to carry out and document the recruitment for the further education course/training and to document the course of the further education course/training; 4) the personal data will be processed on the basis of: a) consent pursuant to Article 6(1)(a) of the General Data Protection Regulation and pursuant to Art.  (b) processing is necessary for the performance of a contract to which the data subject is a party or to take steps at the request of the data subject prior to entering into a contract; (c) processing is necessary for compliance with a legal obligation incumbent on the controller; 5) personal data will be stored:  (a) for the period of participation in the Szczecin Classic Academy plus a period of 5 years counted from the end of the calendar year in connection with the fulfilment of a legal obligation of the Administrator (b) for a period of 2 years from the date of enrolment in the Szczecin Classic Academy and not joining it for the purposes of settling disputes for the assertion of claims 6) The data subject has the right of access to the content of his/her data and the right to rectification, erasure, restriction of processing, the right to data portability, the right to lodge an objection and the right to lodge a complaint to the President of the Office for Personal Data Protection in case the processing of personal data is considered to violate the provisions of the General Data Protection Regulation; 7) Provision of personal data is required at the stage of recruitment to Szczecin Classic Academy; Failure to provide personal data makes it impossible to take part in the recruitment; 8) Data processing is necessary to fulfil the Administrator's purposes 9) Personal data will not be transferred to third countries or third parties, 10) Personal data may be processed in ICT systems, however, your data will not be processed in an automated manner and will not be subject to profiling.
